The Challenge
Physician Housecalls is a premier provider of home-based geriatric primary care services across Oklahoma and Kansas.
At the time of Council Capital’s investment, Physician Housecall’s revenue was entirely traditional fee-for-service. Value-based care (VBC) models, however, offered incentives that could drive better care for patients and better financial performance for the business.
To capture the full potential of these value-based care arrangements, the company needed to develop new skills around payer contracting and data analytics.
The Solution
The Value Creation Team partnered with Physician Housecalls to implement a series of best practices from across the Council ecosystem, including:
- Strategy: Developed value-based care roadmap, and added Board members with deep VBC experience
- Relationships: Leveraged the CEO Council cfor introductions to Medicare Advantage plans
- Reimbursement Models: Entered four value-based care models incl. CMMI’s Primary Care First, MSSP ACOs, Medicare Advantage, and DCE programs
- Analytics: Improved cost and utilization metrics and appropriately recorded HCC Risk Scores
